Online Forms

We’re pleased to offer the following forms for online completion to easily communicate with us and register to our parish, sign up for our programs, and our ministries.

For Adults:

Parish Registration – join our parish or update the information we have on record.

Facility Request Form – request use of a room or other space within the parish grounds. Once the request is approved by staff, the event will be added to the calendar.

RCIA Inquirer Form – sign up for RCIA and join us Mondays at 7pm to learn more about the Catholic faith. Everyone is welcome!

Funeral Mass Selection Form – please complete this form to submit funeral Mass selections for readings, music, and other special requests and needs.

For Children:

Altar Server Application – sign up to be an altar server and enroll in the upcoming training classes.

Baptism Registration – parents can begin the process of having your newborn infant baptized by completing this form. The office will then be in touch with you to make arrangements and notify you of the next baptism class (this one time session is required). For school age children, please register in the Faith Formation program below. Adults seeking baptism should complete the RCIA form above.

Faith Formation Program Registration – register your children in the PREP Faith Formation program. There is an option to pay online through FaithDirect.net after the form is submitted.
